2) Allow password restricted sending to avoid e-mail spoofs (password is in subject and filtered out if correct) Very important. Unfortunately, I am about to write a MTA pickup handler just to avoid this kind of spoofing. It would be nice if a coming release will have this option for list server.

Please consider adding "X-AuthUser" header to messages to log the smtp auth. username of the sender.

To test if the reason is NTFS permissions, I would first check IIS to learn, under which account mewebsite is running. Then give FULL CONTROL to that account on Mail Enable folder and all subfolders. Could it be the insufficient NTFS permissions on Mail Enable data folders?
Check the permissions of IME_ADMIN, IME_USER
or the user who has IIS anonymous access to MEWebMail web site
on the "C:\Program Files\Mail Enable" and all sub folders.
