Activity logging
We use AWStats to log email activity on our server. The reports are fairly comprehensive and very informative.
The whole system was fairly easy to setup and automate.

In my opinion you were way out of line and should be apologising to the staff at MailEnable for your bullying tactics and rude behaviour. Blacklist for Dynamic IP's
It would appear that the majority of spam we receive gets sent from dial-up accounts. Does anyone have any suggestions for a decent RDNS blacklist particularly for blocking dynamic ip's.

This was supposed to be fixed in Pro 1.18 but there was a problem and a hotfix was issued that corrected the problem but caused others. Pro 1.19 works correctly with a size of 0.

Bad mail
I had a similar problem after installing the hotfix to the SMTP service for 1.18. Re-installed the original 1.18 SMTP service and the problem went away.

Spam filter
I currently use MEFilter which is doing a great job. I tried SpamAssassin which promised to teach itself about spam but, as you mention, running in a single thread is painful. Didn't try SNIMTA as I could not see any option to hold mail for eyeball checks.

AVG Command line scanner
Email technicalsupport@grisoft.com and ask for AVGSCANL.EXE, they will send it to you. Make sure you ask for the correct file as they tried to tell me that AVGSCAN.EXE does not support exit codes (which we all know by now) even though I mentioned AVGSCANL.EXE. I did get the file on my second request.
